PANJIM: The GFA affiliated clubs have been asked to submit forms for renewal of club affiliation and participation in official competitions of GFA, form for nominating office-bearer and specimen signature card for the new 2019-20 season.
The last date for submission of forms to GFA is May 15.
It is mandatory for Clubs to submit e-mail address and incomplete forms will not be accepted.
Forms are available at GFA Secretariat, Panjim for Ilhas –Zone Clubs, Duler Stadium, Mapusa for Bardez Zone Clubs, GFA office at Fatorda Stadium, Margao for Salcete Zone Clubs and with Francisco Nunes at Chicalim for Mormugao Zone Clubs.
Clubs have been asked to collect the forms during office hours from April 25.
